Primary hypercalcemia is a condition in which the parathyroid glands which control the calcium metabolism in the body undergo enlargement. Most common is a single parthyroid adenoma in about 80% followed by double adenoma.

Occasionally, there could be enlargement of all 4 glands ( hyperplasia). In about 1% ( very rare) there could be a parathyroid carcinoma. Patients have no visible neck swelling, but they develop swelling in their bones ( Brown’s tumour).

A high level of suspicion and appropriate tests reveal the diagnosis of parathyroid adenoma. The treatment involves surgery of the gland. Astute judgement and appropriate interpretation of tests aid in performing adequate surgery.
